WEB CAMERA CONSULTATION INSTRUCTIONS

We use MSN instant messenger with a contact at info@drlylevh.com. When you log on to MSN instant messenger the clinic staff will be the first to respond.  The staff will tell you if Dr. Van Hemert is available, as this will be worked in between patients.  You may have to wait for the doctor to become free or the staff will be able to set up a time for you to log back on for your consultation.  The web consultation will be limited to five minutes.

If this doesn’t work, you still have the option of coming into the office for a free consultation. You need to have answers to the following questions in advance of the consultation.  The web camera consultation is limited to people 18 and over and living within 20 miles of Sioux Falls, South Dakota.

1.What is your major complaint?

2. How did this condition develop: (What caused it? How and when did it start?)

3. Have you ever had this problem or similar problem before? 

4. Has the problem been getting better, worse, or staying the same?

5. Is there anything that you can do to make it better or worse?

6. Describe your pain. Is it sharp, dull burning achy, stabbing, ect

7. Have you ever received any treatment for this condition?  If yes, when, where, and what were your results?
